Rowan needed a physical exam and drug testing done before enrolling in the pre-college summer program he wants to do. It turns out that the doctor was at the beach a couple weeks ago, and recognized him from the spectacle that ensued from him and his friend ignoring the "no running" sign. He recognized her from the beachgoing audience as well, and got very fidgety and embarrassed.
She understandingly tried to put him at ease during her inspection of his body by making some smalltalk, asking if he's been running at any more beaches, if he's learned his lesson about following rules, commenting on how much whiter his bottom is than she remembered, etc. But, petulant little brat that Rowan is, he just got even more uncooperative, until she had no choice but to put him over her lap and spank him until he agreed to do whatever she said. When she took his temperature the most convenient way available given their current positions, she felt him get rock hard against her thighs; this unacceptable behavior earned Rowan an extra lesson in manners at the end of the physical, with the help of some standard issue hospital equipment she had the nurse bring in.
